If I can get the Word document, I can take a look at it. It should be possible to retain the cross-links, although I usually advise to remove them since they cause headaches very often.
I am also interested to see what happens when I run it through my experimental DOCX to HTML converter which uses a totally different mechanism. I think my advise in general would be, as also mentioned before, is try to use a single level. To be honest I don't really feel the need, but that is probably because I don't really get it. |

Only thing about Kobo Forma (at least the books I've tested on), is it overrides centered <div>s too, so centered images get shifted left. But I consider that a software bug/quirk (and I'll have to report it to them). Again, this should be on Amazon to actually fix the functionality in their renderer, NOT be pulling books down over common CSS/formatting. Updating cross-links in Word is fairly easy. First in the main text, Ctrl-A (Select All) and hit F9. If there are tables (such as table of contents) it will ask if you want to update entire tables or just page numbers. Then place the cursor in any footnote/endnote. Since I used cross-links to refer to real footnotes (which I hid as described before in the Word primary text), and cross-link numbering does not automatically update, you must do this manually. Ctrl-A in the footnotes (Select All for the references) and press F9. Word will warn you that your action cannot be undone. Accept that, and all the cross-links will correctly update throughout the footnotes.
BTW, cross-references are clickable in my version of Word. And yes, some of the footnotes are almost separate chapters. And until the book can be professionally edited, they will remain long, complex footnotes. Using EPUB and what I have learned in this thread will allow me to separate primary and secondary footnotes (endnotes), and hopefully on the printed page as well.
