That book should be KFX since the listing on Amazon says "Enhanced Typesetting: Enabled". To be sure whether or not that format was delivered to your Fire you can open the book and in the "Aa" menu if you can change the "Alignment" or select "Continuous scrolling" then it is definitely KFX format.

Have you checked that this eBook does not have two covers by trying to go back after opening to the cover?

No, there's only the one cover. But interestingly, I see that it is followed by a half-title which has the footer:

Cover Page
Page 1 of 377 * 0%

And the identical footer appears beneath the cover. However, Go To Beginning and (as the first item under TABLE OF CONTENTS) Cover Page both take me to the actual cover, not the half-title page.

Yes, I can change the alignment to left-aligned, so I am looking at the KFX version.

The problem is that if you convert an ePub with an html file to display the cover, then you have to covers. Kindles do not need html to display the cover. The cover is just the graphic image and as long as it's labeled as the cover, then when the conversion is done, the Kindle will know it's the cover and display it. so you are getting the Kindle displaying the cover image and then the html from the ePub displaying the cover image for two covers but one cover image.
