Can you bypass first two items in TOC.ncx?

Hi all!
My client would like their ebook to start at the first chapter, but if the back arrow is pushed, the Dedication and Table of Contents would display.
I've tried taking the Dedication and TOC out of the playOrder in the toc.ncx file, and setting the item ref to linear="no" in the content.opf file, but then the Kindle puts the Dedication and Table of Contents at the end of the book.
I read about the "start" attribute. I understand that the book will then ALWAYS open to the first chapter, not what we want.
Is there a way to do what my client asks? To bypass the Dedication and Table of Contents when the book is first opened, but have them 1st and 2nd in the item order thereafter?

Use the "start_reading" guide item. This specifies where the book will initially open to, and then you can use "page back" to look at whatever comes before that in the book.

Use the "start_reading" guide item.
Does this code belong in the toc.ncx file? Is it an attribute of the playOrder?

Assuming you're using Mobipocket Creator, select the "Guide" page, and add an element with the following properties:

Title: Start Reading
Type: other.ms-firstpage
Filename: book.html#start_reading (or whatever the URL is that you want the book to open at)

Don't forget to click the "Update" button at the bottom of the page, to write the entry back to the file. This adds the entry to the "Guide" section of the OPF file.
