could you find the best setting even if i use text reflow to make it in good shape on kindel the space between lines is not the same and it look bad

The default options work pretty well for me (along with -r to go right to left). You can increase the size by setting dpi to 400 instead of 300, for example, e.g.
k2pdfopt -r -odpi 400 -vls 1.5 mydoc.pdf Your margin cropping is a bit aggressive (it will cut off some of the text on some of the pages) and not really necessary--nor are the -ac or -de options since your scan is very clean. The font varies in size because some of it is from the footnotes, which are typeset in smaller font. The -vls 1.5 should keep the line spacing more uniform at 1.5x single space.
