TOC - Kindle - Not Generating

Hi All,

First off Calibre is brilliant, but I can't get it to generate a TOC at all. Essentially my raw html comes with just <p> tags, but is generally quite clean. The Heuristics pick up the chapters fine, however having read lots of stuff and clicked lots of buttons I can't get it to generate a TOC. Below is the conversion Log, it just looks like it's not doing it! Any ideas would be gratefully recieved!

Enable use auto toc and reset the level 1 toc expression. Level 1 toc should look like the page break before expression after resetting.
