Problem with TOC

I'm trying to make an epub out of several short stories, but I have a problem with the TOC.
I would like the TOC to be of the format

Title of Story
Chapter
Title of Story

The trouble is that all I seem to be getting are the chapters. In Calibre reader all the chapters are showing (but not the titles) but on my reader only the last chapter of the last story is showing.

The titles are styled with h1 and the chapters with h2. What am I doing wrong?

TOC Level 1: //h:h1

TOC Level 2: //h:h2

That works for the calibre reader but not for my reader. Now I'm just getting the title of the last story.
I'm using OpenInkpot and fbreader so I'll ask on that bit and see if it's a problem at their end.
