FYI,
Calibre no longer opens when choosing it from "Open with..." in finder and it no longer opens when choose "Open with..." in a browser "Save" dialog.

Works for me on my mac. Steps I tried:

1) Open Finder
2) Right click on an epub
3) Choose Open with -> calibre (5.99.2)
4) calibre opens and the epub is added to the library as expected.

Yes, when I use "open with" in the finder and choose calibre it launches the app. Iit may just be a Firefox thing but when downloading an epub, choosing calibre does not launch the app but choosing Apple's Books app does.

Also, the "Open with" behavior in finder is not consistent. One time it will launch calibre either with a double-click or a rt-click "Open with". My previous reply was after testing it and it worked. I then closed calibre and tried again and neither the double-click nore the rt-click launched it.

As a matter of fact, you can see that it attempts the launch because the "running" indicator in the Dock flashes on, then off and the launch never completes.
