Cromag instead of using the cart just use the buy now (or similar statement).

The cart is not compulsory - so delete your book from the cart and start the process again.

Also, have you got your credit card details or Paypal already in?

I'm not sure if it's still the case with the new checkout, but you need to make sure that under your account settings, "Show standard checkout" is selected in order for promo codes to show.
