Flghtcrew does a good job checking what it is supposed to for epub2. The op said they use both epubcheck and flghtcrew but that epubcheck did not catch the problem. There is a real issue in the xhtml url as described in the posts above.. The version of flightcrew in master will now at least point out the problem instead of silenting throwing an exception.
