I haven't read the entire thread but is it possible that a connected e-reader is set to receive books in a format different than that in the library? When sending books to the reader, caliibre would save the converted format to the library, AFAIK. (I'm not at my personal computer at the moment, so I can't check this right now.)

I know, but it could be the reason why a format reapers in the library after it was deleted. Maybe there's an option to prevent calibre from storing the format after uploading it to the reader. I haven't checked that either. (I just convert every book to AZW3, and keep it alongside the EPUB. If there is a significant change or bugfix in the AZW3 converter, I just remove the AZW3 books and convert again overnight.)
