Is "Madam" not used as a term of respect at all in the US? In the UK, in any good hotel, restaurant, etc, a woman will normally be addressed as "madam". It's regarded as very polite here. It's the female equivalent of calling a man "sir".

We call heads of major branches of government Secretary. So the head of the Department of State is the Secretary of State. And the current one is female is addressed or referred to as Madam Secretary.

It's used pretty often here - at least in the South.


And by parents in sarcasm before punishments are handed out...
