If you have arthritic hands and holding a reader is going to be an issue, then pressing page turns buttons is also going to be an issue. So a stand will hold the reader and the IR touch will allow fairly easy (compared to physical buttons) page turns.

The only problem you'll have is that you may have to switch from the Kindle as the current model Kindles don't currently have IR Touch. The next model Kindles may have touch and if they do, then you'll be all set with a stand and a new Kindle.
