I have been using the default conversion settings. I have never tweaked them.

Note sure what you mean here. If you click the chapter button on the calibre reader, it will show the chapters in a mobi file. This is always how I test whether the conversion kept the chapters. If it doesn't show any chapters, then the book does not have them. It hasn't failed me yet.
