I did that, and upon connecting again a new file was generated, but a missing book was still missing, while the book I deleted was gone.

This is what I thought as well, but even after a disconnect and a "importing content..." load bar that file does not seem to be updated with the new book when I check on the next connect, even though it's fully readable on the device itself by this stage.

Not sure how to go about fixing this, or if I've got a faulty device somehow. I'm on Calibre 7.2.1 and 4.38.23171 firmware

How are you checking the database? The database is NOT the calibre.metadata; it's the KoboReader.sqlite in .kobo/

Do you have Kobo Utilities? It'll tell you if the KoboReader.sqlite file is malformed when it attempts to backup.

Unfortunately if it is and you don't have an uncorrupted backup you'll have to do a logout-login in the Kobo settings to re-initialize the db. This will re-import any sideloaded books but you'll lose reading positions and annotations that have not been synced to the server (e.g. sideloaded materials).

I've not checked that file specifically, I just figured the book was successfully imported after seeing the Importing Content message and the presence of the book on the device, and I can't figure out why the same book is still missing on Calibre on the next connect.

I've had to do the logout-login and even attempted a factory reset already at this point, because after a bit of faffing about some of these books straight up disappear from my device after the fact.

The bizarre thing is this also happened on my partner's KLC using Calibre on her own PC. She also lost access to her books and needed to do the logout-login. Both our devices are brand new, and we're wondering if there's a faulty recent batch

There's a known firmware glitch where connecting to the PC doesn't always close the database first, especially if you have a kepub book open at the time. Though it's supposed to have been fixed in more recent versions at least one user mentioned it's still happening to them.
