Honestly, I don't know. I thought it was the "bookmarks.png" that is in calibre resources. But, the one there is different. It could be from calibre v2 or something as I did this years ago. I have attached it. Hopefully I won't get into trouble for that.

It might be from a 'Theme' set (user contributed?).
That one looks (nice) a bit different from the style of the current batch.

According to github history, calibre used @davidfor's icon until 29 Oct 2016, or at least something very close to it.

That sounds right. Most of the icons I used are ones in the calibre resources. I just grabbed one as I knew they would work and planned to find something better later. And I'm sure you can guess what didn't happen
