That's a sign that the cover has not been updated in the book properly. As already stated, the Polish books tool and the Modify ePub plugin have ways to fix this. You might need to experiment to get exactly what you want. Opening the book in the calibre viewer will show the results.

The rough steps to do this are:

1. Update the cover in the calibre library. You can use the metadata download to do this, or add the cover manually from some other source.
2. Replace the cover in the ebub. This can be done with the Polish books tool, the Modify ePub plugin, an epub-to-epub conversion or with the editor.
3. Check the results using the calibre viewer.
4. Send the book to the device.
5. Check the displayed cover image.

The only issues with the above are related to badly coded books. But, the methods of replacing the cover should cover this. And if you update the cover in the library later, everything works better.
