I think that Joe is a pretty approachable guy - you can try to contact him with your problem - I think he would seriously try to give you a html version.

Which route did you use, out of interest? I ran the PDF through MobiCreator and while it seems to have picked up all the text, it missed a dozen or so chapter headers and didn't deal well with the multi-line italic sections.

PDF to anything is a huge pain! :-) Pretty much the next step to anywhere is to get to clean HTML, then running it through Calibre should provide a reasonable spread of formats.

You might want to take a read through Jeffrey Carver's experiences in the thread here if/when you have time!

Found it uneventful using Calibre to convert the PDF to ePub.

It's OK, but it misses out the picture of the note on page 2, and all the chapter headings are just text, so no table of contents - it does get the multi-page italics (e.g. in Chapter 4) correct though.
