I don't have a cover, so this will work. If I get a case (which I'd like to do), then I'll have to implement a different tactic, like sticking an adhesive label on the back of the Forma, or on whatever case I end up getting.

Maybe you could create a sleep image that is a photo of yourself holding the reader and a sign that says "this is my Kobo" 😁

It is not a "major fail". It isn't perfect for the reasons stated, but, even when using a sleep cover, the screensaver is shown for a bit after opening the screen. It shows for enough time to see there is some information on it. And it wouldn't take many close-and-opens to get a name and phone number if they are clearly shown. And it isn't hard to work out that pressing the power button would show it.
