Also, not unimportant, the taskbar properties.
Right-click taskbar, select properties and make sure the following setting is made:

As others have said:
  1. Close Calibre. There should only be your original, pinned icon now.
  2. Unpin this icon. You should now have none.

I don't know if you watched the attached video, but that's exactly what I did. I started calibre from desktop icon, then pinned that - resulting in 2nd icon on taskbar. After that I unpinned previously pinned icon, then pinned the running one. Clicking on that one resulted in 2nd icon again.

Restarting the OS didn't change anything.

It's crap like this that makes users stop updating working software.

Not sure if it matters, but you did the actions in a different order to what was suggested.

I would completely uninstall Calibre (AFAIK this does not affect configuration settings or libraries, but good idea to make sure first!) and then reinstall. Or when Calibre is closed, pin to taskbar from the Start Menu entry. I had to do it that way for my browser once.

Well, you could always "upgrade" to Windows 7!

Or, you could move to macOS.

I've checking around and it looks like a Windows 10 problem, and not only with calibre.

I had the same behaviour (on Windows 10) with a couple of other packages before I experienced it with Calibre. Evernote was one and maybe 1by1 or ConEmu was the other.

In all cases removing the existing pinned item from the TB, running the program from the .exe and pinning the 'transient' icon solved the problem - so far for all time.

@thed0g - are you running any taskbar enhancement gadgets, or have you applied any related tweaks. If so try killing/undoing it and see if that overcomes the problem. FWIW, I use 7+ Taskbar Tweaker with no ill effects.

FYI - the Start Menu items are also shortcuts (.LNK files).

For the programs that had the problem:- After unpinning the existing icon, I started them from their .exe files (C:\Program Files\Calibre2\calibre.exe, and C:\Program Files (x86)\Evernote\Evernote\Evernote.exe), and pinned the resultant transient icon to the TB. For some reason repinning the .exe didn't work

I don't have program shortcuts on my desktop, I know the Start Menu is buried deep within the bowels of Program Data somewhere, but I never use it.

FWIW: the taskbar yet another folder of shortcuts, it's stored at %AppData%\Microsoft\Internet Explorer\Quick Launch\User Pinned\TaskBar. Internet Explorer -- it's legacy from the days of Windows 98 and the Active Desktop

Yes, but on my notes, from when I fixed my browser icon problem, I specified desktop shortcuts (I bolded and underlined, so I'm guessing that was an important distinction). To be safe, perhaps best to go from program folder
