If I am watching TV in real time I read during the commercials. I also take my HD+ to the theater with me and read before the movie starts.

I also read in bed to fall asleep. Very rarely do I read a book so exciting that it keeps me awake.
Practicing meditation has taught me to catch wandering thoughts and calm down. Ideally this should be done with full concentration before going to bed. You should try to find a nighttime routine that does not excite you but calms you down. Obviously reading shouldn't be a part of it in your case.

My main problem with reading at bedtime is if it's a real page-turner, it's hard to put down the book and go to sleep. Re-reading an old familiar book is good at bedtime because there's no anxiety over what's going to happen next.

Stop reading just before bed. Seriously. It's obviously interfering with your sleep. Instead get in the habit of listening to some great relaxing music. I'm one of those that can't read just before bed for much the same reason, so I just created a sleep playlist and use an app that automatically shuts the music off after say an hour. Since I also use my Itouch for an alarm clock, it works well.

Usually I read to fall asleep and it works great.

But there are some evil women out there who keep writing books that make me keep going "one more chapter and then I'll sleep" and one more after that.

Evil women like Mercedes Lackey, Elizabeth Moon and Lois McMaster Bujold...

do you use LCD? There is some research, but it is quite at an initial stage, over the relationship between LCD and quality of sleeping/insomnia. Eink should be preferred.

Fictional books are relaxing indeed, complicated books can make indeed your brain thinking while asleep. I guess should be more something related to your private life than the fictional books. With the exception that your habit of insomnia started when you started to read in the bed, so could be present a certain element of novelty, that could easily disturb your brain.

Best remedy: go to run and before to sleep do an activity as meditation.
