He can use a CSS variable to keep the image and text proportions correct in any font size, though I am pretty sure it is not supported on all readers, any reader that supports HTML5 should support this feature.

I don't know what you mean, that's pretty basic CSS which should be supported by most readers (not HTML5):

  vertical-align: -0.7em;
}
(I mean the "height" part, assuming we have already determined that "vertical-align" works.)

I think the previous poster was referring to changing the size of the "alt" text (apologies if I've misunderstood). It's probably not an issue, given that there's really no circumstance in which the "alt" text would be displayed on a reading device.

If the image has fixed dimensions and you increase the font-size, the size of the font can be larger than the size of the image, the proportions would be lost.
I don't see any reason why a reader wouldn't be able to pull the image if your file has no errors either.

@Jellby, your CSS seems fine but I havn't checked it, in any case, my proposal was to use a variable were, for instance. x = 10pt and then set the font-size to x and image height to x * 2 for example, that way, the proportions should preserve.
