Pandoc - suggested by a MobileReader here - does a fair job of inserting smart quotes correctly into ordinary text files - better than some commercial software, at least. The quality of the final product can vary quite a bit depending on the individual author's style though. Usually I find it worthwhile before inserting the text into Word.
Also, when correcting quotes, I've found the Microsoft Keyboard Layout Creator quite helpful to quickly insert single or double quotes, em and en-dashes and ellipses etc., so I have single-open assigned to AltGr+<, single-close to AltGr+> and so on, and the assignments seem to work with most Windows apps that I use.
