I use Delicious Library on my iMac to keep track of all of my reading, be it books I've purchased, library books, audiobooks, whatever.

My sister, now in her 50s, has kept track of what she's read since she first started to read. She just uses notebooks to do so (cheaper than blank books).

I think she remembers because she dedicated herself to the task at 5 years old in honor of our elderly neighbor, who was a substitute grandfather. This does not mean that she never rereads. I remember if I read most books, but with series so common now, I'm sometimes confused as to where I stopped in a series. My other sister, has a phenomenal memory of novels (not so much non-fiction). She can remember pretty minute details of a book she read once 15 years ago.
