You can add an empty book using the dropdown arrow on the add button (or indeed a plugin like Manage Series or Goodreads Sync).

The book will look no different to a book you have formats for in the library view. The only differnce will be it having no formats listed for it on the right on the book details panel. A convention that (many?) people use is to put a tag on the book to indicate it is a "Wishlist" item or similar.

As for what happens when you get the real book, that depends on how you add it (there are so many ways!). Certainly it is possible to add it to Calibre such that the "real" format for that book is just added to the existing "empty" book record in Calibre. It is no different to the issue of duplicate resolution for adding any books. If you add it in the "wrong" way then you will end up with a duplicate book record.

As for being downloaded to your reader - no it cannot be, as there is no book format available to be sent to it. All that will exist is (assuming you populated the metadata) the cover.jpg and metadata.opf file for it.
