But I do have a lot of images and I wonder if that is causing the confusion. In the end epub may not be the best (or even worth the effort?) if the book contains a lot of images.
This is probably true.

Still having issues although about to try deleting the heading styles and trying inserting page breaks.
Just checked the style guide and in respect of headings it says "Word offers various Heading styles, usually for use with the start of chapters or sections. Some of our ebook formats such as EPUB and MOBI will automatically insert a page break before each Heading, which is a nice touch because it allows your chapter to start at the top of the page on the e-reading device."
But it doesn't seem to happen
The style guide says page breaks will be stripped out and ignored - will be interesting to see.

Very strange but changed all Heading Styles back to normal, and inserted page breaks - exactly the opposite of the Styleguide - and now all page breaks display as they should.

I just went through this myself. Manual page breaks are the way to go for Meatgrinder. I think the Style Guide may be referring to an earlier version of the ePub format when it says that page breaks will not be honored.

When I formatted mine exactly how the Style Guide said, it looked awful in all formats. When I went back and added in my manual page breaks, everything suddenly snapped into place and looked great.

I read the other day that Smashwords just put out a new style guide... maybe some of these things are updated in the new one?

Anyway... definitely, definitely definitely use the hard page breaks, and avoid the Heading styles if you can. It's just easier that way, and looks great!

Actually, I put two paragraphs returns after each chapter. Also, I recently read that after the chapter heading, which should be centered, a lot of writers are putting one paragraph return, but at size 28.
