The TOC opens in a sidebar (press the button with 3 lines on a page) in Calibre viewer.
On ADE reading devices, it is called '(Go to) Catalog'
EPUBReader it is a sidebar that may be hidden.

EPUB does not need one (a inline TOC) as you can see.

If you want one, you have to Type it in

If you want it linked , you are going to have to make those HREF type links in code view

After you have finished creating your TOC in Sigil you can convert your epub file to mobi in Calibre and then convert the mobi file back to epub. You will them have an inline TOC in your ebub. Calibre sticks it at the end of the book but you can move it where you want it.

This can cause some minor formating issues so you would want to do this early in your conversion process.

You can also build an in-line html toc from the toc.ncx of the epub.

Here are two solutions. The first uses perl:

and the other uses regex find and replace on a copy of toc.ncx.

The second method codes the TOC entries as an unordered list (<ul>) and visually preserves a nested TOC structure as nested <ul>'s.

once you have the html code, you can put it wherever you want in the ebook. This also avoids any gratuitous reformatting that Calibre might try to do.
