Well I gave up trying to fix the ePub and decided to go back and see if I could fix the original file. In case someone else is having the same problem exporting to CS5 and can only do it without a table of contents or it hangs I'm posting what it was I did that fixed it for me.
I exported the CS5 file to IDXL and opened it in CS4 and was able to save a perfectly good ePub but chapters weren't on new pages because it was all one file. So I opened the CS4 file in CS5 and it no longer hung while creating an ePub with a TOC. Whatever was causing the file to hang was stripped out in the export/import. Though ADE wont display the cover but I think that's a different issue. I'm starting another thread.
