However, I suppose we're getting off the point - regardless if those hidden folders exist - how do I tell Calibre to import its own output correctly?

If Calibre can't resolve this, that's okay - I've manually pushed through reloading those 12000 titles already - so this is less an immediate concern... but if there is a way, it would be nice to know how to import this data properly in the future without all the manual effort.

Nope, calibre does not create such folders.
You need to remove the hidden folders yourself then the remaining folders which are actually calibre output will import just fine.
