Have you tried just using your thumb knuckle to squeeze it (keeping your thumb straight) rather than trying to bend your thumb over to press (which is what I'm assuming you're doing if your thumb is cramping)? I find that works really nicely and lets you hold it one handed securely and without thumb fatigue.

I'm a rightie, but I almost always hold the Kobo in my left -- like Psyke's second picture. So the button for me is actually in the correct spot.

I assume the Kobo also has the ability to rotate the textby 90 degree?

While not an ideal solution, this should make it easier to hold it left-handed.
