I've seen this behavior in DRM'd PDF's before as well; i.e., allowing printing, etc., though the few I've seen that allowed printing usually limited the output to 300dpi. Have you tried running it through PrimoPDF? If that doesn't work you can try "updating" print drivers for a real printer to use the drivers supplied by you PDF printer and possibly trick it.

got it to finally work.

1. install primoPDF
2. select "print to file" (directly printing to primoPDF produces a blank page)
3. save it as something.ps
4. ps2pdf something.ps something.pdf doesn't work - gives an error about the postscript being created from an encrypted file
so i opened something.ps in ghostscript and printed via primoPDF

now i can finally read a book i paid for in the first place *sigh

Sweet! Oh how the mighty have fallen, yet another chink in the DRMonster's hide. Pretty novel approach you took to open it in ghostscript and then print. We'll have to add this to our work-around methods.
