Books in calibre and device despite being the same

When I add books to my Kindle PW (using send to...), in some cases the book shows up as not in device. For instance, I send the book "Night Fall" to my Kindle. I switch to device view, it's there (as "John Corey - 03 - Night Fall"), but the green checkmark is missing. I switch to library view, it's there (as "Night Fall"), but again the green checkmark is missing.
Also, this seems to happen only with books that are part of a series (but not in all such books).
What's going on?

Did you change the title or use the Plugboard to inject the series info?

Whatever you are doing, Calbre now thinks there is a different copy of the book on the device

I use the plugboard to insert the Series info into the 'Title' (This is not the same as inserting series into the filename)

My guess is that either the .metadata.calibre file on the device is corrupt or is being deleted, or that the books are being sent from a different library than is being used to look at the device. In both cases calibre falls back to title/author matching and it isn't finding a match in the library for the book on the device.
