My suggestion (I don' have a technical answer!) is to re-post this query in the Reader Content forum, with a more specific title (which includes something like "Table of Contents for html docs?"). You'll probably get more responses this way.

I would use Book Deisgner for converting HTML into LRF.
BD will allow you to create TOC in two ways. Automotically and linked TOC or both
