Failure to correctly convert apostrophes, etc.
Calibre has decided to stop converting symbols such ', en and em dashes, etc.

I have installed the latest version of Calibre, and run with the settings detailed above. I then reset to defaults, and achieved the same result.

Edited to add: this is a conversion of a web page, filtered.

I take it you tried with unchecking Transliterate Unicode to ASCII? Otherwise, follow the post that Kovid Goyal linked to. The more information you can supply, the more likely that the problem will be located.

Paste Web page into Word or LO Writer.
Fix anything that needs fixed (save / edit odt in Writer)
Import a docx into Calibre.
Works perfectly.

I'm doing multiple books a week with Calibre to epub from docx and also mobi or azw3. No problems.
