FWIW, you can get very very close by using "OpenWith" to open your CSS stylesheet into its own editor window alongside the editor's own xhtml and preview windows. Editing in either/both is possible with the css changes appearing in Preview when your css editor writes to disk.

I do this in Sigil all the time, but I am very sure Calibre can do the same thing.

Thanks @KevinH, that is a good suggestion. I hadn't noticed that option previously. I can open and edit with Notepad++ and then Calibre even asks whether you want to import after saving the changes.

I'll use that method.
