manifest and spine issue

i have an ebook (epub) with font embed. all work fine, and my ipad read ebook correctly.
but on calibre, on check error, i have 2 error: manifest and spine are missing on opf file. but i already have on my opf file!
start <manifest> at line 12 and end </manifest> at line 57
start <spine toc="ncx"> at line 58 and end </spine> at line 95

another issue is cover: all ok on my epub, but put on my ipad, ipad don't see cover! (from library page). but when open my ebook i see correct cover image.
and also i have only one cover.xhtml at first place on my xhtml files, but saved book and re-open on calibre i have another cover.xhtml at the end of the list with ! red symbol. delete this file, reopen book i have again same problem....

suggestion?

I suspect there is a parseing error (missing or wrong symbol or punctuation)
in the OPF. Probably above or in Manifest

Validate is NOT supposed to be forgiving of common errors that render engines allow.
{ ) ; : ones are really hard to spot

solved!
after careful analysis on my ebook on opf file is missing </metadata> before manifest section.
add this, all work correctly! no error found.

now i have another little problem: my very stupid ipad don't see cover image on library section.
but cover is normally dislayed when my book is open on ipad...

- try to rename name of book on opf page: the same;
- (re)-mark cover.xhtml as cover: the same;
- on library is dislayed as unknow author... but on opf i have <dc:creator xmlns:ns0="http://www.idpf.org/2007/opf" ns0:role="aut">author name</dc:creator>

suggestion?

i have forgotten to mark (on image group) cover.jpg as cover!
