Customizing the toolbar

Hello,
I read an old thread that described how to add "Copy to Library" to the toolbar. I have tried this several times and the new item never appears on the toolbar however I can add it to the menubar successfully.

Is there some step that I have missed or misunderstood?

There are 2 versions of toolbars
Without device
With device connected

IMHO some things only make sense for one of those

Also depending on the number of buttons and their size, it may have gone into the overflow section. See if there's some tiny dots by the rightmost button and click on it if there are.
