DRM removal not working

How do I remove DRM from a book already in Calibre? I have a book in my Calibre library which was added before the DRM plugin was installed. The book was from B&N and is still available to me from B&N and in my Nook library. I have already deleted it from my Calibre library and tried to add it again from my Nook Library, but Calibre wants to know if I want to add a duplicate, like it were still there. I can't read it on either Calibre, my computer, or my tablet. I tried in Nook and it wants the credit card # and email. (both have changed and I don't know the old ones).
