mobi and pdf heading fonts incorrect epub ok

Today I uploaded a .docx file to calibre 3.20 64-bit version and created pdf, mobi and epub files. The epub file looks great. However, the mobi and pdf files have monospace/courier new font for the chapter headings (h1) instead of the times new roman font that is in the .docx file for chapter headings (h1).
I have other files I created in calibre and this does not happen in those files. I've checked the settings of my current document and made sure they match my previous documents that didn't have this bug. I've uninstalled calibre and reinstalled a previous version, reinstalled the latest version, checked and rechecked settings in calibre but cannot find a fix for this. I've also searched google, duckduckgo and mobileread forums but haven't found a solution.
Can anyone help?

I created a bug report (#1758696) and included samples of the problem.
