Buttons to advance chapters on Kindle

I have a Kindle 4 with the forward/back buttons on the side. In converting a mobi file, in this case a novel using chapter numbers, those buttons aren't moving chapter to chapter (the TOC only shows a Prologue). What is the setting that allows them to do so - or do I have to modify something else? Under Heuristic Processing, I've got everything checked, but that hasn't done it. Thanks,

You need a proper TOC for them to work.

I found an epub copy with a TOC that I converted to mobi, but it had what looked like triple or quadruple spacing between paragraphs. I tried the heuristic option to delete all blank lines, but that was overdoing it; I still wanted one blank line. Is there a way? In the meantime, to get some reasonable spacing, I left the heuristic choice deleting all the blank lines, and added the Look & Feel Layout option inserting a blank line, which seemed to leave a little more than the heuristic method alone. Is there a better way, i.e., to get a single blank line between paragraphs in a situation like this? Thanks,
