How do I add a file to the spine?

I have a document/epub book in which the Editor reports:

"This file is not in the book spine. All content documents must be in the spine. You should probably add it to the spine."

How do I add the file to the spine?

What is the file(name)?
Where does it lay (section) in the files browser list?

Sorry for the delay - holidays, family and reading.

the file is:
ops/xhtml/contents.html

And is listed in the Miscellaneous folder

I noticed that the latest update has a feature - automatically resolve not in spine files. Before I try it does anybody know whether it just deletes the file or what?

Thanks eschwartz. It pays to update as this was not on my version when I first posted.
Now at 2.15 and there it is.
