When converting from epub to fb2, is there any way to specify using <title> </title> tags for each chapter rather than the tags currently used? Or at least keep the original <H?> used in the original epub?

No, I don't think there is, but user_none who wrote the FB2 Output plugin is the best person to confirm/deny.

No. Calibre has a very flexible system for marking chapters and any chapters created in the FB2 output would need to honor them. Unfortunately, I don't have the time or desire to implement this functionality.

At one time it did create chapter boundaries with <title> </title> using H tags but it was removed because 1) it was confusing to people why their custom TOC's were not being honored and 2) not all books mark chapters with a single type of H tag.
