help need for omnibus file?

Thanks to HarryT and Patricia, I've just managed to make a Book Designer file for a book I wanted for my Sony 505. I'm delighted with the program itself and with the tutorials; this is a very helpful place!

This particular author -- Annie Fellows Johnston -- has 13 books in a series.

If I wanted to make an omnibus file of all 13, is there a way to get the chapter titles on a different level than the book titles? and how do you make the "book indexes" that are in some of the omnibus titles I've downloaded from here?

As you can probably tell from the question, I'm a good end user, but not a programmer ... if it's way complicated, I'll just live with 13 separate files.
