Generate catalgue problem

I have a problem when i generate a catalgue (last version calibre) with authors list with letter A /Æ and D/ Ð

No problem during the generation but when i open the epub with the viewer i can see double entry (with same link) for theses letters in the TOC ...

A precision : i use in parameter for hierachy presentation
Divided by fist letter
Combine letter with number < 0
Minimise if more than 200 elements (enough for 26 letters and other none ascii letter )

Just an another request :
The list propose authors and all theirs books but without indentation
Do you think it's possible to add an option or to indicate where to change something if it's possible
Example

Authors 1
...Book 1
...Book 2
...Book 3
Authors 2
...Book 4
...Book 5

And not this presentation less visible

Authors 1
Book 1
Book 2
Book 3
Authors 2
Book 4
Book 5

Unfortunately the person that wrote the catalog code is no longer active so it's in maintenance mode not seeing any new features/developement.

At least for the second request I can offer an idea. I use different symbol for each book whether it is read or unread or stopped. Due to that all books starts with a symbol followed by title and the infirmation I like to see.
The symbol can be added in the catalog creation dialog.
Let me know when you need more help on this.
