How do I fix unmanifested files?
I have a free eBook with a placeholder cover. I found the actual cover image in the ePub, but since it's not in the manifest I can't set it. How do I fix this with Calibre?

Calibre puts a cover icon in the book browser. The HTML file that calls that image, must also be set to cover. If there already is a Cover file, just edit the image tag to be the real image flagged above. Another way is to simply copy and past the image into the MDE and Polish once the edit session is done.
