Remove or move custom metadata fields

How do I hide or move custom metadata fields from the Edit Metadata window?

If you're on a recent version of Calibre, go to Preferences > Look & Feel > Edit Metadata.

If you're on an older version, there's a tweak that lets you rearrange them, but you can't hide them aside from an option in the Job Spy plugin (and its implementation means the contents become uneditable).
