Metadata persistence.
When i add metadata to a book via Calibre, is that metadata written to the file, or is it just a part of the database?

@procyon - just the database.
You can update the actual book files (formats) via the Polish Book tool (EPUB and AZW3 only), and most formats via the Embed Metadata tool, the Modify optional plugin (EPUB only) can also be used. Polish and Modify can do other things - e.g. font subsetting, removing proprietary artefacts etc. BR |

Your original questions answer: To the DB only.
Books sent to devices, get updated on the fly BR's answer makes the Library copy of the book reflect the change. |
