Fonts and layout changing during conversion

First off, I LUV Calibre! Kudos on an extremely powerful and elegant tool.

I could really use help with an .epub that is one large chapter with no
.ncx table of contents. I want to split it into several chapters, and create
a .ncx table of contents. I converted it in Calibre from .EPUB to .EPUB, and
it made the chapters and the TOC, but the fonts and the layouts were changed.
Is there any way to convert the file, but make no changes to the layouts and fonts? I Disabled font size rescaling, embedded all fonts, expanded CSS (see attachments). Should I use any other options?

Is there a way to split a file or create a .ncx toc without conversion?

Edit the file with the Calibre editor (tap T)
splitting (single) is done in the Preview pane. The icon with the horizontal line. Then move the green line to where you want to split and click.

Any way to create a TOC that is visible outside Calibre?

In the Editor or Main Calibre, first create the NCX TOC (it is the 3 lines and finger icon)
Once that is done (in the Editor)Tools:Table... : Insert inline TOC

It uses the NCX to generate the HTML code

You can edit/style this page (does not affect the NCX)

I simply right click on the book and choose Edit TOC. Now I click on the button Generate TOC from files. Edit the TOC entries as required and click OK.
