Question Why is the .doc still a .doc?

I was trying to read a book on my Kindle Voyager that is in a .doc format. The format is a mess and it is very distracting. Using Calibre, I tried converting the ebook into both a .mobi format as well as .azw3. In Finder, I see the ebook in the converted formats. But when I send the converted formats to my Kindle, they still show up as a .doc file.

Did you use Send to Device?
Each device driver has a configuration of which formats and format priority (for when multiple suppurated are present). You can change these (and do it wrong )

With the device connected: On the Device icon: Configure

For newer Kindles AZW3 is my top, followed by Mobi, followed by other AZW

@ChipAHoy - I didn't think calibre knew about DOC. But I see it's in the preferred input list but not the Use Viewer for list - maybe that implies calibre doesn't know how to convert DOC to MOBI or AZW3.

If you can get the 'book' into DOCX format with Word or something, and provided its a well formed DOCX, then if you convert that into MOBI or AZW you should get a better result.

The old DOC format is an awful format to start from, as bad or worse than PDF. If I didn't have Word, I would convert it to RTF (there are online converters that will do that), knock it into shape with LO Writer, save it as DOCX, and convert that.
