Footnote formatting

Can anyone explain to me how footnotes should be formatted? I'm experiencing some funny stuff on my Kindle. Normally I put all footnotes in a single file at the end, with the text linking to the footnote and the footnote linking back to the text. Usually this works fine, and a single footnote pops up when I click the link, with a "go to footnotes" button at the bottom of the pop-up window. However, I sometimes see the entire footnotes file displayed in the pop-up, and I can't figure out what is making this happen.

Can anyone tell me how to make sure footnotes are treated separately please?

OK, that fixes it -- but in most cases I have all the footnotes in one file and it works fine, and I'm damned if I can see why it doesn't always work! And I prefer to have them in one file, to avoid clutter, and so that I don't end up with each footnote on a page by itself... Anyone have any ideas?
