Italics jumping paragraphs

I'm converting a document from ePub to HTMLz.
There's lots of paragraphs in italics interspersed with paragraphs in regular.
And after conversion, most of the italics paragraphs are in regular with the following regular paragraph in italics. I had this problem converting from PDF, which is a mess anyway, but now I'm seeing it in ePub conversions.

Is this a conversion of Anything that started out as a PDF?
PDF documents are not linear when different fonts or images are included.
(Think: paint by numbers. Place regular text. Place pictures. place Italic text.)

I don't know if this ePub was converted from a PDF.
I also have three PDFs and an RTF of the same document and while the RTFs and PDFs have matching contents, the ePub is completely different ... cleaner.

I only asked, because from PDF (anytime in the books history) conversions are messy and hard to recover the original intent.

(x)HTML based originals are cleanest. Mobi is a lower level of HTML based.
